Diabetes.co.uk Prescription Service is a partnership between Diabetes.co.uk and Pharmacy2U. Pharmacy2U, who are the UK's largest NHS contracted pharmacy* and specialists in the direct to patient delivery of medicines for 17 years (*NHSBSA, April 2016) also respect your privacy. Pharmacy2U will receive your registration details and no medical or medication data received from you is shared by Pharmacy2U with any third party, including Diabetes.co.uk.
Terms and conditions
I understand the Diabetes.co.uk Prescription Service and Pharmacy2U's services and wish to register for their use. I understand that Pharmacy2U will receive my registration details from DCUK for the purpose of providing the Prescription Service and that no medical or medication data is shared. I understand EPS nomination and nominate Pharmacy2U Ltd to collect my prescriptions on my behalf either via EPS or directly from my GP. I understand that by submitting this form I give consent for my prescriptions and information about my repeat medicines to be sent electronically between my doctor and Pharmacy2U.
