*Terms and conditions apply. You must be aged 16 years or over, resident in the UK, and have diabetes, to enter this prize draw. If you are a parent or guardian of someone under 16 years old who has diabetes or are a carer of someone with diabetes, you may enter on their behalf. By submitting the form, you agree that we can process the information you have entered about your medical condition and frequency of testing to run this prize draw and, if you have agreed, to send you product information. Sometimes we decide to give away even more free meters, and we may process your data to determine eligibility for this too. You must not submit an entry using someone else’s data unless you have their express permission. We do not accept entries sent through agencies. Entry to the prize draw must be made through the entry form on the Ascensia Diabetes Care promotion page on Diabetes.co.uk. You do not have to pay anything to enter. This prize draw runs from 1 August 2017 until 28 February 2018. You can only enter this prize draw once. You can only enter one prize draw for a CONTOUR NEXT ONE meter in any 18 month period. If you enter and we see that you have already entered a prize draw in the last 18 months, we will ignore your entry. At the start of each week, we will pick 50 winners at random from the previous week’s prize draw entries, and each winner will receive a CONTOUR NEXT ONE blood glucose meter. There are no alternative prizes. If you are picked as a winner, we will send a notification to the email address on your entry form within 15 days of the draw. We will send out the meters within 28 days following the draw, to the address on your entry form. We do not accept any liability for technical failures of any kind including internet failures which may occur in the administration of entries. For a winners' list, send a stamped addressed envelope to “Free Meter Prize Draw” to us, at the address below, although please do bear in mind that we will not provide any sensitive personal data in our reply. If you are one of our employees, or a relative of an employee, you are not eligible to enter. Sorry! By entering this promotion, you agree to these rules, which are subject to English law. “We” refers to Ascensia Diabetes Care, Ascensia House, Albert Road, Newbury, Berkshire, RG14 1DL.
